CANCELLATION POLICY
The Consulting Clinic reserves the right to charge the full fee due in the event of a missed appointment or an appointment cancelled with less than 48 hours notice.
REFUNDS
Please contact the clinic for enquiries related to refunds. The Consulting Clinic is under no obligation to offer a refund of any fees paid but will be reviewed after the request.
CONFIDENTIALITY
Therapy appointments are highly confidential, although there are legal exceptions i.e. if you pose a threat to your own or other’s safety then confidentiality may be broken. Before the disclosure of such information, it would be discussed with you as the client.
If you wish to share any personal information about your mental health with a third party, your full consent needs to given. You will appreciate that due to the level of confidential information which we hold, we will require your full informed and unconditional consent, prior to arranging any meeting on your behalf or sharing or releasing any information to your designated third party.
DATA POTECTION ACT
We store all patient personal details in accordance with the Data Protection Act. All clinical notes and reports remain the property of the psychologist at The Consulting Clinic. Copies of notes can be made available on request. The Consulting Clinic reserves the right to make an administration charge for any requests related to copies of files. Clinical notes are stored securely and are encrypted on an online software called Wellola.
EXTRA WORK UNDERTAKEN
Requests for reports will be billed at the hourly rate. GP reports are free of charge.
CONSENT FORMS
Prior to the assessment and commencing of treatment, all clients will be provided with the clinic’s Terms & Conditions. The Clinic requires that all clients that commence treatment at The Consulting Clinic sign the consent form and terms & conditions.
